If our facility is out of network with your insurance provider, please call our billing department and discuss the situation with us.

If you have a PPO plan, you will have both in and out of network coverage. If we are out of network with your insurance plane, we will do our best to offer a facility fee that is comparable to an in-network provider. In many cases, there may be other benefits available to you.

Please bring your card. At the time of registration, we must make a copy of your insurance card.

Your copay amount is due on the date of your procedure. We then submit the bill to your private insurance firm. After this, a bill for outstanding services is sent to a secondary insurance firm or sent to you with a balance due.

Medicare

We do accept assignment of benefits through Medicare. A bill is sent to your secondary insurance after payment or denial from Medicare. If you don’t have a secondary insurance, the bill is sent to you for balance due. We make a copy of your Medicare card and secondary insurance card when you register. If you have signed your Medicare rights over to a separate insurance company, we require a copy of that card at the time of registration.

Self-Pay Cosmetic Surgery/Elective Surgery

We will contact you prior to your surgery with an estimate of costs for your surgery. Payment is required on the day of or before the date of service.

On the day of your service, payment may be required for the anesthesiologist’s fees. Someone from our billing department will call you to discuss what these may be.
